(“FVUMC”). Utilize all required safety equipment, including but not limited to safety glasses, gloves, and appropriate footwear. Immediately report any unsafe conditions or potential hazards to the project leader. Never perform work that you deem to be unsafe or for which you have not received adequate instruction. Adhere to all safety protocols regarding the use of ladders, electrical tools, and any other potentially dangerous tools or situations. Scope of Work: Only perform tasks assigned by the project leader and within the agreed-upon scope of the project. Do not expand the work without explicit authorization. Do not perform any work that requires a professional license (e.g., electrical, plumbing) unless you possess the appropriate and current license, and have provided proof of such to FVUMC. If an issue requiring a licensed professional arises, immediately stop work, secure the area, and inform the project leader. Quality of Workmanship: Perform all tasks to the best of your ability, demonstrating a commitment to quality and attention to detail, consistent with the instructions provided. Ensure all repairs and installations are durable, functional, and visually acceptable, considering the limitations of the volunteer setting and available resources. Use supplied materials in accordance with best practices, and according to the directions given by the project leader. Leave the work area clean and free of debris upon completion, and ensure that all tools and equipment are properly stored. Supervision and Communication: Follow the direction of the project leader. Maintain clear and open communication with the project leader and other volunteers. Promptly report any problems, delays, concerns, or changes in the work to the project leader. Report any incidents, accidents, or injuries immediately to the project leader, regardless of severity. Tool and Equipment: Only use tools and equipment that you have been trained to operate safely. Maintain tools and equipment in proper working order, and inspect them before use. Report any defects to the project leader immediately. Use tools and equipment only for their intended purpose. Documentation and Inspection: Complete any required documentation related to the work performed. Understand that all completed work is subject to inspection by the project leader or a designated FVUMC representative. Be willing to address, in a timely manner, any deficiencies identified during the inspection, to the extent of your ability and within the scope of the project. Professional Conduct: Treat recipients of services with respect, compassion, and sensitivity at all times. Maintain a professional demeanor while on the work site, including appropriate language and attire. Respect the recipient’s property and privacy. Do not access areas of the home beyond those necessary to complete the assigned tasks. Adherence to Guidelines: Follow all verbal and written guidelines provided by the project leader and FVUMC. Participate in any required safety briefings or training sessions. Criminal Convictions: Do not participate in this ministry as a volunteer if you have been convicted of any felony during the 10 year period preceding the date you are scheduled to volunteer or if you have been convicted of any crime in the preceding 10 years which is a violent crime (e.g. assault), a sex offense, a crime against vulnerable persons (e.g. child, person with a disability, or senior citizens), or a crime that involves the unlawful taking or conversion of another’s property (e.g. larceny, shoplifting, or embezzlement). If you are a minor, your parent or legal guardian previously named should sign. Scope of Relationship. I desire to provide volunteer services for Fuquay-Varina United Methodist Church, Inc. (“FVUMC”). I understand that the scope of my relationship with FVUMC is limited to a volunteer position. No compensation will be provided in return for services provided by me, nor will I be entitled to any employee benefits. I understand that I am responsible for my own insurance coverage in the event of personal injury or illness as a result of my volunteer services to FVUMC. Release and Waiver. I hereby release and forever discharge and hold harmless FVUMC, its successors and assigns, its volunteers, and their respective partners, agents, operators, managers, employees, and representatives (“Released Parties”) from any and all claims I may have or hereinafter have for any injury, illness, temporary or permanent disability, death, damages, liabilities, expenses and/or causes of action, now known or hereinafter known in any jurisdiction in the world, attributable or relating in any manner to my service as a volunteer, whether caused by the negligence of the Released Parties or by any other reason. I acknowledge and agree that this Volunteer Release and Waiver of Liability is intended to be, and is, a complete release of any responsibility of the Released Parties for any and all personal injuries, illness, temporary or permanent disability, death, and/or property damage sustained by me as a result of my volunteer services. No Insurance Required. I understand that FVUMC is not required to provide medical, health, or disability benefits or insurance, nor is FVUMC required to provide any financial or other assistance, in the event of injury, illness, disability, death, or property damage related to my volunteer services. Medical Treatment. I hereby release and forever discharge FVUMC from any claim whatsoever which arises or may hereafter arise on account of any medical treatment or services, including first-aid or similar services, rendered in connection with my volunteer services. Assumption of Risk. I understand and acknowledge that participation in volunteer activities involves certain risks, including but not limited to property damage, serious injury, illness, temporary or permanent disability, or death. I understand that I may be asked to engage in hazardous activities such as construction, loading and unloading, heavy lifting, transportation to and from the work site, and other similar activities. I understand that in the event of injury or illness, neither the volunteers nor FVUMC are required to provide medical services, and any injury or illness sustained may be compounded by negligent or delayed medical service. I AM VOLUNTARILY PARTICIPATING IN SUCH ACTIVITIES WITH KNOWLEDGE OF THE DANGER INVOLVED, AND I AGREE TO ACCEPT ALL RISKS OF PARTICIPATION, INCLUDING THE RISK OF INJURY, ILLNESS, TEMPORARY OR PERMANENT DISABILITY, DEATH, OR PROPERTY DAMAGE. Indemnification. I agree to indemnify and hold harmless FVUMC for all claims, accusations, notices, judgments, rulings, liabilities or expenses arising out of my actions, inactions, errors, acts, or omissions as a volunteer. Covenant Not to Sue. I agree, for myself and all my heirs, successors and assigns, not to sue the Released Parties or initiate or assist in the prosecution of any claim for damages or cause of action against the Released Parties which I or my heirs may have as a result of any personal injury, illness, temporary or permanent disability, death or property damage I may sustain as a result of my services as a volunteer. Publicity Release. I hereby grant to FVUMC all rights, title, and interest in and to any and all photographic images and video or audio recordings made during my volunteer activities with FVUMC, including but not limited to any royalties, proceeds or other benefits from such photographs or recordings. I grant permission to use my image in print, video, web, social and/or digital media by FVUMC. Miscellaneous. I understand that this document is a contract which grants certain rights to, and eliminates the liability of, FVUMC and the other Released Parties. This Volunteer Release and Waiver of Liability shall be governed by the laws of the State of North Carolina. This Volunteer Release and Waiver of Liability is intended to be as broad and inclusive as permitted by law. In the event any provision of this Volunteer Release and Waiver of Liability shall be held to be invalid by any court of competent jurisdiction, the invalidity of such provision shall not otherwise affect the remaining provisions of this agreement, which shall continue to be enforceable.
